About NICU Grad

NICU Grad is a virtual-first care provider offering wraparound, post-discharge follow-up care for NICU graduates and other infants with complex medical and social needs.

The Problem We're Solving

Each year, more than 10% of the nearly 4 million infants in the United States are born premature. Compared to their term counterparts, premature infants have complex social, developmental, and medical needs following discharge from the neonatal intensive care unit (NICU).

Unfortunately, the coordinated transitional and follow-up care required is difficult to access. These gaps in care lead to worse outcomes for NICU graduates, significant strain for families, and costly, avoidable care.

Over a third of NICU graduates are hospitalized (compared to less than 10% of term infants) and half present to the emergency department within their first year of life.

Our Approach

Our multidisciplinary care team—including nurses, social workers, and lactation consultants—partners with families prior to NICU discharge and is accessible 24/7 for support.

The team works in collaboration with patients' primary care providers and subspecialists to coordinate care, secure resources for health-related social needs, provide individualized education, and facilitate group and peer support.

The duration of NICU Grad services ranges from 3 months to 3 years, tailored to individual patient and family profiles.

What's Next

The NICU Grad platform will eventually include additional remote monitoring capabilities for patients requiring nasogastric feeding or supplemental oxygen, further extending our ability to support families with complex needs at home.
